Use this page to register your organization and get a Subscriber ID (User ID) to simplify access to participating healthcare sites.

Who should register:
Only your organization’s OneHealthPort Administrator should complete this registration. The Administrator manages the account and can add or update users. If you are not the Administrator, please contact the correct person within your organization.

Registration steps:

1. Create Profiles - Enter basic information about yourself and your organization.
2. Review Agreements - Review and accept the required terms and conditions.
3. Verify Identity - Complete identity verification with OneHealthPort.
4. Confirm Registration - Look for an email confirming your registration that includes your Subscriber ID and instructions to create your password.

Please allow up to 72 business hours for us to review your registration after completion.
